Utensils must be stored handles up, run through dishwasher to avoid contaminating eating surfaces when putting away. Both freezers' holding temperature at 2 F; must hold 0 F or below - provide accurate thermometers. Cutting board on steam table needs replaning/replacing.

Floor sink needs cleaning. Harvey's Resort Casino natural spring water bottled water not from approved source. Label must contain a permit number from the Nevada State Health Division. Product should be pulled.

Provide a supply of sanitary paper towels at hand-washing sink. Store wipe cloths in a container of sanitizing solution; i.e., 50 ppm chlorine, 200 ppm quarternary ammonium. Properly label packaged sandwiches with location, ingredients in descending order. Provide thermometer in refrigerated display case.

Two Martini & Rossi brand vermouth bottles were voluntarily disposed of for adulteration with fruit flies.

Line cook was observed to be picking up raw hamburger with hand. Complex chef (supervisor) had to instruct employee to wash hands before handling ready-to-eat food to prevent cross contamination. Employee needs training. Cooking-line hand-washing facilities lack supply of sanitary towels and/or hand cleanser

Buffalo chopper cover had rough welds that do not meet NSF standard. Unit was replaced. Rough sealant application on die case condensation pan should be properly finished as to be smooth. Repair leaking hot water faucet. Store raw oysters below ready-to-eat food, i.e., sliced tomatoes. Chipped and cracked wooden salad bowls should be discarded. Recommend using more durable plastic bowls.

Cuisinart food processor was for household use only and needs to be replaced with NSF approved type. Buffalo chopper had rough, possibly toxic welds on cover and needs to be replaced. Replace insulation cover in ice machine door. Floor sink needs cleaned by walk-in cooler. Replace ceiling light shield over pot-washing area. Employee noted with tongs in back pants pocket. Potato ricer discarded for rust and toxic welds.

Cold plates must be integrally connected to ice bin. Beverage tubing must not contact ice used for human consumption. Cold plate should be relocated outside ice bin.

Bar gun head has red slime; remove spout and clean routinely. Prime sanitizer fee line when changing out bleach chlorine. Test sanitizer routinely. Terminate equipment drain lines at least 1 inch above floor sink rim. Run bar gun holster, drain line to floor sink. Do not place in sink drain.

Floor-service glass washer rinse sanitizer non-detectable. Discontinue use and disable unit until repaired. Floor-service bus station hand-washing facility blocked by trash containers. Provide soap and paper towels at sink by coffee machine.

Provide a supply of hand cleanser and sanitary paper towels. Clean floor sink of thick black mold. Glass washer final rinse sanitizer non-detectable. Discontinue use and disable unit until serviced to provide 50 ppm chlorine sanitizer. Keep half-and-half dairy product at 40 F or below. Product voluntarily disposed of.

Install double check valve vented to atmosphere on carbonator downstream of copper water supply line. Eliminate all downstream copper piping to prevent copper from leaching into product.

Glass washer rinse sanitizer nondetectable. Food line may have been pinched. Unit was working after lines were moved. Test sanitizer concentration routinely with approved test kit. Floor needs to be cleaned at wall juncture. Electrical cords obstruct proper cleaning methods. Equipment drain lines must terminate at least 1 inch above floor sink. Clean floor sink.
