Carson City Health Department's food-service inspections for Aug. 5. All scores are on a 100-point scale, with points deducted depending on the severity of violations:

• Playa Azul Restaurant, 415 E. William St., scored 96 points. Cloths used on food-contact surfaces were found stored in buckets without detectable sanitizer residual. This was corrected on the spot. The hood over the grill had some grease drips large enough to fall onto the grill. Floor tiles were broken or missing in some areas especially coving tiles. Walls and ceilings had some holes and accumulated dust which could fall and contaminate food. The dining window is broken and must be replaced or repaired.

• Kim Lee's Sushi & Teri Restaurant Main Kitchen, 319 N. Carson St., scored 98 points. Food was found stored for later use which was inadequately labeled or unlabeled. The outside garbage/ refuse storage van was leaking onto the street. This liquid appeared to be an oil and did not have a garbage odor.

• Kim Lee's Sushi & Teri Restaurant Oyster Bar, 319 N. Carson St., scored 99 points. There was no "No Smoking" sign at the entrance door.

• Kim Lee's Sushi & Teri Restaurant Sushi Bar, 319 N. Carson St., scored 100 points.
