SPECIALIZED HEALTHCARE PROVIDER SERVICES

Fallon Paiute-Shoshone Tribe

The Fallon Paiute-Shoshone Tribe, on behalf of the Fallon Tribal Health

Center, is requesting proposals from qualified individuals/firms for the

following specialized healthcare providers:

Cardiologist PSYCHIATRIST (general/adult)

Endodontist PSYCHIATRIST (child/youth)

OPTOMETRIST

Please contact Jon Pishion, Health Director, by email at fthcdirector@

fpst.org, to receive a complete RFP package. Responses are due by

Friday, December 2, 2022, by 5 pm PST. Proposals not received after

that time and date shall be rejected.

In accordance with 25 CFR and Tribal TERO Ordinance, Indian Preference

will be afforded to qualified Native American applicants/firms. The

Fallon Paiute-Shoshone Tribe reserves the right to accept and or refuse

any and/or all responses.
