The certificate of manager-level food safety training was unavailable or expired. Cleaned and sanitized equipment and utensils, laundered linens, and single-service and single-use articles were being stored in an unapproved location where sources of contamination were present. Test strips weren't readily available to measure sanitizer concentrations. The plumbing wasn't properly sized, installed or maintained. Staff members weren't able to use one of the sink's three basins due to a major leak, hindering their ability to manually wash dishes.

The certificate of manager-level food safety training was unavailable or expired. The restaurant's methods for cooling food were inadequate to achieve rapid cooling. The inspector saw chicken being placed directly into the cooler after being cooked. Frozen and potentially hazardous food was being thawed in a manner that doesn't minimize microbial growth. Bacon and chicken were being left out to thaw. Floor, wall or ceiling surfaces were dirty. The surfaces under the line equipment needed to be cleaned. Potentially hazardous food was being held at temperatures that support micro-organism growth.
