Carson City Health Department's food-service inspections and the date of inspection (all scores are on a 100-point scale, with points deducted depending on the severity of violations):

Through Oct 30

- Bordewich, Grace Elementary School, 110 S. Thompson Street, school cafeteria, fee exempt, scored 100 points.

- Central Warehouse, Carson City School District, W. Telegraph and Richmond Street, school cafeteria, fee exempt, 100 points.

- Carson Middle School, 1140 W. King Street, school cafeteria, fee exempt, scored 100 points.

- Carson High School, 1111 S. Saliman Street, food facility, sports field concession stand, fee exempt, scored 95 points. There is no sanitizer available for sanitizing utensils or setting up sanitizer buckets for storing wiping cloths in. There are no chemical test strips to measure the concentration of the sanitizer. Remove plywood being used as a food prep surface (for coffee pots) positioned over the middle of the 3-compartment sink.

- Carson High School, 1111 S. Saliman Street, school cafeteria, fee exempt, gymnasium snack bar, scored 99 points. Found no thermometers in two refrigerators.

- Carson High School, 1111 S. Saliman Street, food facility fee exempt, Culinary Arts Kitchen, scored 99 points. Coving is missing from the floor/wall contact behind the ice machine.

- Mustang Sallie's Deuce's Grill, 420 N. Carson Street, restaurant, scored 98 points. There was no certified food handler at the establishment during the time of this inspection.

- Cactus Jack's Senator Club, 420 N. Carson Street, bar/lounge, scored 98 points. A supply of sanitary towels were not present at the hand wash sink.

- International House of Pancakes, 3883 S. Carson Street, restaurant, scored 100 points.

- Pizza Hut No. 011662, 2441 N. Carson Street, restaurant, scored 98 points. Found cooked sausage on the pizza make-up line at 45.7F.

- Molly's Catering and Gourmet, 220 W. John Street, take out, caterer, scored 94 points. Found front reach-in unit at 46-50F. Found two other reach-ins at 42F. Thick dust is accumulating on the ceiling over the main food prep area and also in the wall ventilation ducts (over the fax machine). Windows and window sills need cleaning in the rear food prep area and in the dishwashing room. Shelving holding clean dishes in the dishwashing room is "sticky."

- Smith's Food and Drug, 599 E. William Street, produce department, scored 96 points. Some nozzles used to spray vegetables contained encrusted deposits and are not clean. The hand washing sink was blocked in the back room from easy access.

- Doppelgangers Bar and Grill, 302 N. Carson Street, kitchen, scored 93 points. Found chicken salad at 52F and cooked chicken pieces at 50F on the cookline refrigerated prep unit. Both main ice machines need to be cleaned more thoroughly.

- Jack in the Box No. 7320, 3225 Retail Drive, restaurant, scored 94 points. Chlorine residual of less than 50 ppm is detected on dishware after mechanical final rinse. Non-food contact surfaces that are subject to splash or food debris are not readily accessible for cleaning or are not clean. Single service articles are not handled, dispensed, or stored in a manner that prevents contamination.

- B & E Catering, 1020 Mountain Street, mobile food vehicle, catering vehicle, scored 100 points. Establishment is clean and well maintained.

Through Nov. 1-7:

- Carson Station, 900 S. Carson Street, restaurant, coffee shop, scored 95 points. Readily perishable cold food is held above 46F. Steak was found in the right drawer refer under the grill at 46F or more. The drawer did not appear to be shut completely to insure that the temperature is maintained at 40F or below.

- Carson Station, 900 S. Carson Street, restaurant, snack bar, scored 95 points. Readily perishable cold food is held above 46F in the reach-in refrigerator. Hamburger and hot dogs were at an internal temperature of 48F. This meat was discarded on the spot. The refrigerator did not seem to be working properly.

- Tommy's Neighborhood Bar, 260 W. Winnie Lane, bar/lounge, scored 97 points. Food contact surfaces of inside the ice machine are not clean. Non-food contact surfaces of equipment of utensils are not clean. A build-up of slime was protruding from the bottled drink ice bin drain tube into the floor sink.

- Toads, 306 E. William Street, bar/lounge, scored 99 points. Cleaning and maintenance tools are not properly stored or are not properly maintained. The mop bucket was found filled with dirty water.

- Carson City BBQ Co., 5110 S. Edmonds Drive, caterer, mobile catering, scored 98 points. Cutting board surfaces are cracked, cut or chipped.

- Winnie Discount Liquor and Grocery, 262 W. Winnie Lane, convenience store, scored 93 points. No soap or paper towels available at hand sink. Hand sink blocked by bottles and cans to go back or that are damaged. Boxes are on the floor blocking access. Ice bags without a proper label identifying location of facility. No sanitizing cloths or sanitizer available for use on surfaces used for single service ware (food contact surfaces).

- Sak'N Safe Food Store, 2100 E. Highway 50, produce department, scored 98 pints. There is no Certified Food Manager on duty in this department.

- Sak'N Safe Food Store, 2100 E. Highway 50, bakery, scored 94 points. Found live ants in a small pool of syrup leaking from the soda box rack. No Certified Food Manager is on duty at this location. (Repeat Violation)

- Sak'N Safe Food Store, 2100 E. Highway 50, deli department, scored 95 points. Found tortillas stored under meats in the sandwich make-up unit at the front service counter and inside the kitchen walk-in. The cook stove in the kitchen has grease build-up on the top and sides. The white plastic storage containers opposite the cook stove need cleaning. (Repeat Violation) There is no Certified Food Manager on duty at this location. (Repeat Violation)

- Sak'N Safe Food Store, 2100 E. Highway 50, retail grocery, scored 97 points. The stand-alone cold cheese carousel unit and the cold sliced meat display do not have easily located thermometers. The thermometer above the egg display is reading 50F. The actual temperature of the eggs is approximately 43F. The ice machine in the back storage area (used for icing the seafood cases) is leaking and smell at the floor level.

- Sak'NSave Food Store, 2100 E. Highway 50, meat department, scored 87 points. Found the 3-compartments sink set up improperly. Dirty wiping cloths used to clean the meat cutting tables are being rinsed out in the 3-compartment sink sanitizer basin instead of the sanitizer buckets, leaving the basin very greasy and murky and useless for sanitizing clean utensils. Found a spray bottle of Windex stored next to spices on a shelf in the seafood service room. There is no thermometer visible in the meat case to the right of the fresh seafood display. Found Styrofoam meat trays above the meat prep tables with blood splatters. (The room temperature is between 43F and 53F. This temperature is not low enough to prevent bacterial growth on the clean trays.)

Unnecessary poisonous or toxic materials are on the premises, specifically: lotions and gel sanitizer on a food prep surface. In-use dispensing utensils are not stored properly. Cloths used on food-contact surfaces are not clean, do not contain sanitizer residual, or are not used properly.

- Silver Dollar Casino, 1897 N. Edmonds Drive, restaurant, Sky Cafe, scored 93 points. Employees do not wash hands thoroughly or as often as necessary to remove contamination. Soap dispenser not working. Employees using unapproved sanitizer in place of proper handwashing. Food contact surfaces of equipment or utensils are not clean. (Specifically: slicer)

- Taco Bell No. 3, 400 S. Carson Street, restaurant, scored 98 points. The cloths used on food-contact surfaces in buckets at the end of the cook line did not contain sanitizer residual. The caulk at the front hand-sink is discolored and cracked.

- Java Girls, 3667 S. Carson Street, take out, scored 100 points.

- Bethlehem Lutheran Church, 1837A N. Mountain Street, school cafeteria, scored 100 points.

- Mark Twain Elementary School, 2111 Carriage Crest Drive, school cafeteria, scored 100 points.

- Empire Elementary School, 1260 Monte Rosa Street, school cafeteria, scored 100 points.

- Fritsch, Edith W. Elementary School, 504 Bath Street, school cafeteria, scored 99 points. In dry storage room, found clean towels stored against food products (rice, bag of flour) and clean utensils (baking sheets) stored among food products.

- Seeliger, Al Elementary School, 2800 S. Saliman Street, school cafeteria, scored 100 points.

- Fremont, John C. Elementary School, 1511 Firebox Road, school cafeteria, scored 99 points. Found sanitizer buckets set up with only the Quat concentrate and the wiping cloths. (Repeat Violation)

- Eagle Valley Middle School, 4151 E. Fifth Street, school cafeteria, fee exempt, scored 99 points. Area around grease trap on the floor under the 3-compartment sink is unsealed and uncleanable. (Repeat Violation)

- Carson Donut, 3170 S. Carson Street, No. 10, bakery, scored 83 points. Found personal foods, pills, handcream and clothing stored improperly. Food service staff has long, unrestrained hair. Staff person is eating personal food in the food service area. No hand-washing was observed at this inspection. Found kitchen hand sink blocked with baking sheet pans, cleaning cloths, and paper. Inspector was directed by the staff person to use the 3-compartment sink to wash hands. (Repeat Violation). Containers of dry foods (gray plastic buckets of flour, sugar, etc.) are not labeled. Several lids are cracked. The 3-compartment sink is not properly set up to wash-rinse and sanitize. Stem thermometers are used in the refrigerator units. No test strips are available for testing the chlorine sanitizer solutions. Numerous wet wiping cloths are laying about the kitchen on food prep surfaces, and in handsink. No sanitizer buckets set up to store the wiping cloths in. No paper towels provided in the staff/public restroom. The ceiling fan in the restroom is not operational. No certified food manager is on site at this facility.
