action for damages incurred as a result of

personal injuries.

after service of this Summons, exclusive of the

day of service.

a. File with the Clerk of this Court, whose

address is shown below, a formal written

answer to the Complaint, along with the

appropriate filing fees, in accordance with the

rules of the Court.

b. Serve a copy of your Answer upon the

Attorney whose name and address is shown

below.

Unless you respond, a Default will be

entered upon application of the Plaintiffs and

this Court may enter a Judgment against you

for the relief demanded in said Complaint.
