NOTICE IS HEREBY GIVEN that the undersigned is the duly appointed and qualified Trustee of the CAROLYN GUIBOR REVOCABLE FAMILY TRUST, under trust agreement dated December 18, 2003, as amended and restated on September 5, 2014 (the “Trust”). CAROLYN GUIBOR, a settlor of said Trust, died on December 18, 2023. A creditor having a claim against the decedent, or the Trust estate must file their claim with the undersigned at the address given below within 90 days after the first publication of this notice.
