The Fallon-Paiute-Shoshone Tribe, on behalf

of the Fallon Tribal Health Center, is requesting

proposals from qualified individuals/firms for the

following specialized healthcare providers:

CONNECTIONS PROJECT

EVALUATOR

HRSA/RURAL

COMMUNITIES OPIOD

RESPONSE PROGRAM

PROJECT EVALUATOR

by email at fthcdirector@fpst.org, to receive a

complete RFP package. Responses are due by

Monday, December 11, 2023 by 5:00 p.m. PDT.

Proposals received after that time and date shall

be rejected.

In accordance with 25 CFR and Tribal TERO

Ordinance, Indian Preference will be afforded to

qualified Native American applicants/firms. The

Fallon Paiute-Shoshone Tribe reserves the right

to accept and/or refuse any/or all responses.
